How NPs ask an insurer for a higher rate

A step-by-step plan: find your number, compare, write the letter, follow up.

  1. Find your current rate. It is on your fee schedule or remittance (ERA/EOB). Use the rate for your most common code, usually 99214.
  2. Compare it. On your state page, see the typical rate for that insurer and Medicare. Check my rate gives a verdict in about 30 seconds.
  3. Write a short, factual letter. State your current rate, the typical rate for your state and code, and a specific ask. The free rate letter drafts it; it never asks for less than you are paid now.
  4. Send it to provider relations or contracting. Keep a copy and note the date.
  5. Follow up. Many insurers take 30–90 days. If they say no, ask what would change their answer; see when they say no.
