Where these numbers come from
Every number is a rate an insurer lists in its public price files, or a public Medicare or Medicaid fee. We don't estimate or survey insurer rates.
Sources
- Insurer rates. Transparency in Coverage machine-readable files published by each insurer (2026 Q4 files). We match each billing group (tax ID) to the NPIs it bills for.
- Medicare. CMS 2026 Physician Fee Schedule. NPs are paid 85% of the physician amount; we show the range across the state's payment areas.
- Medicaid. Each state's 2026 fee-for-service schedule (office, adult). Where a state publishes no NP rate we show the physician rate and say so. Managed-care plans can pay differently.
- Platform pay. What NPs report a platform pays them per visit. Never tied to a name.
- Practice environment. AANP 2026 State Practice Environment map.
How we keep it reliable
- Typical rate is the median across billing groups of the same type. We show it only when 5 or more groups list the visit.
- Middle half of groups (25th to 75th percentile) appears only with 10 or more groups.
- Rates under $1 are dropped as file errors. Insurers that don't sell in a state are left out of that state.
- "Mostly one fee schedule" means 80% or more of those groups share one fee schedule, so the number reflects one contract template.
- Reported platform pay is published only once 5 different NPs report the same platform, state and visit; outliers more than 40% from the median are dropped.
How we keep out ghost rates
Insurer files list prices for every code a contract covers, including codes and providers that never see a patient. Here is what we do so those "ghost" rates don't set the numbers you see.
- Only NP billing groups count. A rate counts only when the insurer's file ties it to a billing group (tax ID or NPI) we can match to individual nurse practitioners in the national NPI registry (NPPES). "Solo NP" means the group bills for one NP and nobody else.
- Only the visits NPs bill most. The main pages use five codes: 99213, 99214 and 99215 follow-ups, the 90792 psychiatric intake and the 90833 therapy add-on.
- Small cells stay hidden. Fewer than 5 groups for an insurer, state and visit means no typical rate is shown at all, and the top 25% figure needs 10 or more groups.
- Each refresh replaces the last. When insurers post new files we reload them, so a group dropped from a file drops out of the numbers. Every page shows which quarter's files it uses.
What listed rates are not
A listed rate is the contracted amount before copays, deductibles and denials. It is not what was paid on a claim. Your own contract governs. Nothing here is legal, billing or financial advice.
Who makes this
NP Link Rates is part of NP Link, an NP Collaborator company. NP Link builds free business tools for nurse practitioners, starting with this one. NP Collaborator helps nurse practitioners find collaborating physicians. The rates shown here come from public files and are the same whether or not you ever use NP Collaborator. Meet the NP who built it.
